For a Glamorous Gaze – Question #1: Are there treatments for dark circles around the eyes?

Dr Tremblay: First we must identify what is the type of eye circles by a thorough visual and tactile examination. Indeed, there are different types of dark circles and treatment differs depending on the type. Some circles are due to genetic, hereditary factors such as dark pigment circles linked to skin color and ethnicity. In this case, the skin is genetically programmed to produce more pigment around the eyes. Intense pulsed light laser treatments are effective, but the results will not last long since the skin is programmed to produce pigment it will continue to do so and more treatments will be needed. We usually recommend treating it with a cream containing high concentrations of vitamin C known for its brightening properties. Of course, the concentration and form of vitamin C is important in the context of an effective treatment. OTC products do not contain a high enough concentration of vitamin C to be effective in the case of pigmented dark circles.

Dr Tremblay: In the specific case where the creases are hollow, meaning that there is a loss of volume, the treatment will be different from pigmented dark circles. Over time, the already very thin and delicate skin around the eyes loses collagen and hyaluronic acid, responsible for volumes around the eyes. We must therefore, if we want to restore volume, inject hyaluronic-based filler. These injections will erase the creases and so minimize the appearance of severe and tired aspect of the eye contour. Filler injections for creases are often combined with Botox injections to treat crow’s feet.

Dr Tremblay: Skin that has thinned out is a common complaint from patients at the first evaluation. The skin around the eyes is constantly solicited by 10,000 blinks per day. Dr Tremblay: All areas of the face where volume losses give a tired, stern and older appearance. Often, what people notice most about others in terms of skin aging are not necessarily wrinkles but rather hollows, sunken areas that are caused by the loss of volume on the face. The purpose of filler injections is to restore face volumes natural structures. Hyaluronic acid can also fill fine lines and wrinkles such as the nasolabial folds and marionette lines starting from each corner of the mouth down to the chin, and lip contour.  Of course, it is the treatment of choice for those who want to add a little volume to their lips.

Question filler injections #2: What are we injecting exactly?

Dr Tremblay: We inject a hyaluronic acid gel. Hyaluronic acid occurs naturally in our body and is used to maintain the moisture level of our skin by capturing and holding water in the deeper layers. Once inside the skin, the gel retains water and mixes with natural hyaluronic acid in the dermis. The benefits of hyaluronic acid on the skin are both corrective and preventive. Maintaining facial volume helps prevent tissue slackening and sagging structures.

In addition, we also know that hyaluronic acid is a biodegradable product that removes naturally in the body.

 

Question filler injections #3: How long does the result of filler injections last and will my skin be worse if I decide to stop the injection treatments?

Dr Tremblay: Hyaluronic acid is a molecule which resolves. Its effects are temporary and last an average of 12-18 months depending on the injected areas. . We must therefore return for touch-ups if we want to keep the rejuvenating effects obtained by hyaluronic acid. However, 60 to 70% of residual benefits are observed even after 12-18 months for the hyaluronic acid has a biostimulative action which stimulates collagen during the time it is in the skin also contributing to repair skin imperfections such as wrinkles and scars. Results are different from one person to another, depending on age, on the treated areas, the degree of sun exposure and if we are smoking or not.

Other advantages are the reversible effect of hyaluronic acid based fillers and the fact that it is less scary than the permanent results of cosmetic surgeries.

Question #1: What is the use of Botox in the treatment of facial rejuvenation?

Dr Tremblay: As part of a cosmetic treatment, Botox is mainly used in the upper part of the face (forehead, glabella, crow’s feet), but sometimes it can also be injected in the chin, neck and jaw muscles according to patient needs. Botox is mainly used to correct and prevent expression wrinkles and sagging eyebrows caused by excessive contraction of facial muscles. Botox is injected directly into the muscle at the location of the wrinkle to relax the voluntary or involuntary contraction of the muscle responsible for the wrinkle.

Dr Tremblay: The effects of Botox last an average of 4 to 6 months. Patients will therefore return two times a year to redo the injection if they wish to maintain the effect of rejuvenation of the upper face. (forehead, glabella and crow’s feet). Following the first treatment of Botox, if patients do not wish to receive it again, the skin gradually returns to baseline.
